Sinemet is a combination of drugs Carbidopa and Levodopa which is a brand name for this combination of medicine. Sinemet is used to manage symptoms of Parkinson's disease, which includes shaking, tremors, stiffness, or movement problems. Parkinson's disease is arise by little amount of Dopamine (a naturally occurring substance) in the brain.

Mechanism of action of Sinemet:

Levodopa is a central nervous system agent. It functions by converting to dopamine (a neurotransmitter in the brain that affects movement and emotions) in the brain, which helps to control movement.

Carbidopa stops the breakdown of Levodopa in the blood flow so that more Levodopa can go into the brain. Carbidopa can also lessen some of Levodopa's side effects like nausea and vomiting.

Contraindications of Sinemet:

  • Avoid the use of this medicine in an individual who are oversensitive to Sinemet or its active constituent.
  • This drug is strictly contraindicated with Nonselective monoamine oxidase (MAO) inhibitors such as Selegiline HCL. You should discontinue the use of these inhibitors at least 14 days before initiating therapy with Sinemet (Carbidopa + Levodopa)
  • This medication is contraindicated in patients with narrow-angle glaucoma.
  • It should not be used in patients with suspicious, a history of melanoma as Levodopa may trigger a malignant melanoma.

Sinemet Drug interactions:

Sinemet may interacts with the following medications such as antiseizure medicines, antacids, antispasmodic medications, Isoniazid, blood pressure drugs, high-protein foods, tricyclic antidepressants Amitriptyline, major tranquilizer like Risperidone, Pyridoxine (vitamin B6), tranquilizers Alprazolam or tricyclic antidepressants Amitriptyline.

You should not take this drug along with MAO inhibitor like Phenelzine and or the Parkinson's drug Selegiline; you must stop it at about 14 days before starting to consume Sinemet.

Dosage information of Sinemet:

Sinemet is come into two form of tablet i.e. extended-release tablet and disintegrated-release tablet. The oral extended-release tablet which is normally consumed two to four times a day. The disintegrated-release tablet is generally consumed three or four times a day. This medicine can be consumed with meal or with empty stomach. Don't crush, chew, or crack the extended-release tablets. Consume them whole.

To consume the orally-disintegrating tablet, take away it from the bottle with dry hands and instantly place it in your mouth for quick dissolution of tablet. You don't require water.

Side effects of Sinemet

Some common side effects of this medication may arise while using this drug which includes nausea, dry mouth, dizziness, diarrhea, constipation, weakness, vomiting, loss of appetite, change in taste, headaches, Confusion, forgetfulness, nervousness, difficulty in sleeping, abnormal dreams, nightmares, weakness drowsiness, and  increased sweating.

Precautionary measures:

  • Before consuming this medicine, inform your doctor your health history, especially liver disease, renal disease glaucoma, asthma problems, heart disease (like heart attack, irregular heartbeat), stomach ulcer, blood disorders, mental or mood disorders like (depression, schizophrenia), seizures and sleep disorders.
  • Do not drive or perform any activity that requires concentration while taking this medicine as it may make you dizzy or drowsy.
  • Avoid alcoholic beverages with this drug

During pregnancy, this medicine should be used only when clearly required. Talk about the hazards and benefits with your doctor.
